We are honored that your student has indicated a desire to participate in the high school interscholastic athletic program.
To get started, please have your child complete the below forms and turn them all into our Athletic Office located by the Fieldhouse. These forms can also be picked up in our Athletic office as well.
In addition to academic eligibility, a physical, completed forms, and facility fee payments are all necessary to compete on a Grand Haven Athletic Team.

Sports Physical Form Can Be Picked Up From High School Athletic Office And Filled Out Prior To Clinic.
