Forms


A number of forms are available on line. You can download them, print them off, fill in the appropriate information and turn into your school building. All are saved in Adobe Portable Document Format (.pdf)

**Special Note**
If enrolling an elementary student during the summer months (June - August) please call 850-5000 to find where and when you can drop off your student's enrollment documents. High school and Middle school students can enroll at their respective buildings during office hours. To properly place your student in high school classes, please provide a current transcript, proof of residency (If not a school of choice applicant), IEP (if necessary), custody/guardianship papers, and immunization records at the time of your enrollment appointment.

Forms are broken down into several categoreis:

  • General Forms- Forms relating to the district as a whole, enrollment or may be used by more than one building level. 
  • Elementary Forms- Forms relating to the elementary buildings.
  • High School- Forms relating to Grand Haven High School
  • Athletic Forms- Forms relating to athletes, teams, and parents of athletes.
  • Child Services/Open Door- Forms relating to our before and after school child care programs.
